Create a paid shipper order

Create one paid ship-only order. external_ref is the idempotency key. An exact retry returns the same HTTP 200 receipt and creates no second order; reusing the reference with different data returns 409. SKUU retries transport errors, 429 and 5xx responses using the identical signed body. Other 4xx responses are permanent.
